What works in practice

• Write down the product and its batch. Not "the usual adhesive" but the brand and batch number. If there is a reaction, you know straight away what was used.
• Re-check health questions at every visit. New medication, a pregnancy or a new allergy changes what you can safely do.
• Book the next appointment on the card. Infill, refresh or the next patch test: if it is written down, nobody has to remember.
• Keep the cards locked away. Health details are special category data under data protection law. This template is not legal advice.
